Congratulations on your acceptance
to the
2011 New Jersey Governor's School in the Sciences program!

We require that all accepted scholars and their families complete the Scholar Commitment Form and the Parent/Guardian Commitment and Permissions Form and return these to us in US Mail. All forms must be postmarked by April 12 in order to ensure your place in the 2011 program.

To Scholars: To declare your commitment to attend this summer's program, please check each box on the Scholar Commitment form to indicate you understand the commitments you are making and sign and date the form.

To Parents/Guardians: We ask that you also check each box on the Parent/Guardian Commitment and Permissions Form to indicate you have read and understand each item. Please complete the bottom of the form including your signature, your address and your child's name.

To Scholars and Parents/Guardians: Should you decide not to attend the program, we ask that you and your families also complete both forms and indicate your intentions accordingly.

Forms should be returned to us in US Mail and postmarked by April 12.
